CountyCare is a health plan that provides coverage to a population that has never been eligible for Medicaid benefits. It is a program administered by Cook County Health and Hospital System (CCHHS).

CCHHS delivers care through its two hospitals, 16 community based clinics, the CORE Center, the Cook County Department of Public Health and to thousands of individuals detained at the Cook County Jail and the Juvenile Temporary Detention Center.
